This Consumer Health Data Privacy Policy supplements our Privacy Policy and applies to personal data defined as "consumer health data" under the Washington State My Health My Data Act (MHMD) and Nevada Senate Bill 370. If you are not a resident of Washington or Nevada, our general Privacy Policy governs your personal information.

Zera Health is committed to respecting your privacy and handling your health data responsibly. This policy describes how we collect, use, and protect consumer health data, and explains the rights available to Washington and Nevada residents under applicable state law.
What Is Consumer Health Data?
Consumer health data means personal information that is linked or reasonably linkable to a consumer and that identifies the consumer's past, present, or future physical or mental health status. Examples include information about an individual's:
- Health conditions, treatments, diseases, or diagnoses

- From third parties such as licensed healthcare providers, compounding pharmacies, and our clinical platform partner (Telegra)
Purposes for Collection and Use
Our collection of consumer health data is necessary to provide our telehealth services. We may also use consumer health data to:

- Right to Deletion: Request deletion of your consumer health data
- Right to Withdraw Consent: Withdraw consent to the collection, sharing, or selling of your consumer health data at any time
- Right to Appeal: Appeal any decision we make in response to your rights request. If your appeal is unsuccessful, you may file a complaint with the Washington State Attorney General at www.atg.wa.gov/file-complaint
Supplement for Nevada Residents
This section supplements the above disclosures for Nevada residents under Nevada Senate Bill 370.
All consumer health data rights described above apply to Nevada residents. In addition, Nevada residents have the right to review and amend any consumer health data we maintain about them.
Nevada residents may escalate unsuccessful appeals of consumer rights requests to the Nevada Attorney General at ag.nv.gov/Complaints.
Third parties may collect consumer health data over time and across different websites when you use our Website in Washington and Nevada.
